ABSTRACT

Clinically curable adrenal metastasis is rare. We report a case of synchronous solitary adrenal metastasis from rectal cancer in a 51-year-old man who underwent curative resection. A right adrenal mass was found by ultrasonography during his routine physical examination and this was confirmed by computed tomography [CT]. His serum carcinoembryonic antigen [CEA] level was found elevated, and colonoscopy revealed a rectal tumor located 10cm from anal verge. A simultaneous laparoscopic right adrenalectomy and anterior resection for rectal carcinoma was performed. Histopathological examination revealed well-differentiated rectal adenocarcinoma with adrenal metastasis. The patient is still alive and free from disease 6 years after the surgery. A review in the literature showed that synchronous solitary adrenal metastasis from colorectal carcinoma is very rare. Surgical resection and for selected patients, laparoscopic procedure may provide survival benefit and potential surgical cure for a solitary metastasis

ABSTRACT

Ectopic prostatic tissue is an underreported entity, which is found most commonly in the lower male genitourinary tract, and ectopic prostate tissue outside the urinary tract is even rarer. Our patient was a unique case of ectopic prostatic tissue within submucosa of the rectum. The patient presented with rectal bleeding, and a firm, round solid submucosa nodule found in the anterior rectum at digital rectal examination, it was 1cm in diameter and 5cm above the anal verge. The size and submucosa location of this nodule were confirmed by the colonoscopy and MRI. After being removed surgically, the histopathology of the specimen sections possessed typical prostatic acini and stroma, meanwhile the immunohistochemical staining for prostate specific antigen confirmed its' prostatic nature. It is the first case to date, which involves the mural of rectum. We hypothesizes that the etiologies of ectopic prostatic tissue within the submucosa rectum attribute to embryogenetic abnormality

ABSTRACT

Implantation metastasis of colorectal cancer in an anal fistula is very rare. We report a case of a 61 years old male who underwent fistulectomy for an anal fistula. Histopathology unexpectedly revealed adenocarcinoma in the fistula track, however the patient refused further treatment. Sixteen months later he presented with an obstructing locally advanced rectosigmoid cancer found to be fixed into the pelvic wall. An ileosigmoid bypass was fashioned and he was treated with neoadjuvant chemoradiotherapy followed by high anterior resection. Histopathology confirmed a colorectal adenocarcinoma, Immunohistochemistry of the tumors from both sites was CK7-/CK20+. The patient died 34 months later with liver and lung metastasis however no perianal recurrence occurred. Local resection with or without radiotherapy, instead of abdominoperineal resection, was feasible for control of perianal metastatic lesion implanted from colorectal cancer if local extended resection was possible

ABSTRACT

AIM: To explore the mechanisms of resistance to 5-fluorouracil(5-FU) in human colon cancer cells.METHODS: 5-FU-resistant cell lines were established and their IC50 were calculated by detection of cell survival rate.Western blotting was performed to analyze the expression of several proteins,by which the possible mechanisms of acquired resistance to 5-FU were determined in human colon cancer cells.RESULTS: The resistant cells were resistant to 5-FU-induced S phase arrest as well as the expression of DNA damage marker-phosphor-histone H2A.X.Furthermore,data demonstrated that over-expression of Bik,Bcl-Xs,and Bcl-XL proteins were observed in 5-FU-resistant cell lines.However,the DLD1/Bcl-XL cells were only partially resistant to 5-FU-induced apoptosis,but not 5-FU-induced S phase arrest and phosphor-histone H2A.X.CONCLUSION: Over-expression of Bcl-XL protein certainly contributes to acquired 5-FU resistance in human colon caners,but has no effect on 5-FU-induced DNA damage and cell cycle disorder,suggesting that other mechanisms are involved in acquired resistance to 5-FU in human colon cancer.

ABSTRACT

AIM: To explore the influence of Chrysanthemum morifolium Ramat on TNF-related apoptosis inducing ligand (TRAIL)-mediated apoptosis in human colon cancer cell line DLD-1 and its possible mechanism. METHODS: Adenovirus-mediated TRAIL gene AD/hTERT-gTRAIL was applied either alone or by combination with Chrysanthemum morifolium Ramat in human colon DLD-1 cell line. Cell growth and apoptosis were measured by inverted microscope, MTT method and flow cytometry. The expression of TRAIL mRNA, TRAIL-Rs mRNA and TRAIL protein expression after exposure to Chrysanthemum morifolium Ramat were measured by semi-quantitive RT-PCR and FACS, respectively. RESULTS: The suppression percentages and apoptotic rate of DLD-1 by Ad/hTERT-gTRAIL alone were 31.4% and 13.5%, respectively. Combination of TRAIL gene transfection with Chrysanthemum morifolium Ramat, the suppression and the apoptosis rate raised to 93.1% and 45.4%, respectively (P
